Privacy Policy

We keep this website simple on purpose: no tracking pixels, no advertising cookies, no online forms. This policy explains what little information the site handles, how we use what you choose to send us, and the rights you have as a California resident.

Who we are and what this policy covers

Health PRTCL · Hyperbaric Medical Group (“Health PRTCL,” “we,” “us”) provides physician-supervised hyperbaric oxygen therapy at one location: 10375 Washington Blvd, Culver City, CA 90232.

This Website Privacy Policy applies to www.healthprtcl.com and to emails or phone calls you start from it. It does not describe how we handle medical records and other health information created when you seek or receive care. That is covered by our separate Notice of Privacy Practices, described in the next section.

Patient health information and our HIPAA Notice

Health information you give us as a patient or prospective patient (for example, your diagnosis, referral notes, insurance details or treatment history) is protected health information. It is governed by the federal Health Insurance Portability and Accountability Act (HIPAA), the California Confidentiality of Medical Information Act (CMIA) and other applicable laws, and by our Notice of Privacy Practices.

The Notice of Privacy Practices is a separate document. It explains how we may use and disclose your health information and your rights to access, amend and receive an accounting of it. Copies are available at the clinic, and we will mail or email one free of charge on request: call (209) 670-1202 or write to info@healthprtcl.com.

If this Website Privacy Policy and the Notice of Privacy Practices ever differ about health information, the Notice of Privacy Practices and applicable health-privacy law control. Nothing here limits your rights under HIPAA or the CMIA.

Please don’t send sensitive medical details by regular email

Standard email is not a secure channel. When you first contact us, include only your name, the best way to reach you and a brief, general reason for writing (for example, “consult request” or “patient referral”). Please do not email Social Security numbers, insurance ID numbers, medical records, imaging or detailed histories unless our staff has set up a secure method with you.

Referring physicians and their staff: please call (209) 670-1202 before sending clinical notes or imaging and we will arrange a secure transfer (for example, secure fax or encrypted email).

Information we collect

Information you choose to give us

The site has no forms, accounts, online booking or patient portal. We only receive personal information if you contact us by email or phone using the links on the site. That may include your name, email address, phone number, the content of your message or voicemail and, for referrals, the referring practice’s details.

Information collected automatically

Like any website, ours is delivered by a hosting and content-delivery provider (currently Cloudflare). To serve pages and protect the site from abuse, that provider processes standard technical data in server and security logs, such as your IP address, browser and device type, the page requested, the referring page, and the date and time of the request. We do not use this data to identify individual visitors.

Cookies, local storage and analytics

  • No advertising or tracking cookies. We do not use advertising networks, social-media pixels, session recording or cross-site tracking.
  • No third-party analytics today. The site does not run Google Analytics or similar tools. If we add privacy-respecting, aggregate traffic statistics in the future, we will update this policy first.
  • Language preference. When you choose English or Español, your browser saves that choice in local storage (a setting named prtcl-lang) so other pages open in the same language. It stays on your device and is not sent to us. You can clear it at any time in your browser settings.
  • Security cookies. Our hosting provider may set a strictly necessary cookie if it needs to tell people apart from automated attack traffic. These cookies are not used for marketing.

Fonts, images and the map

Our fonts, photographs and the clinic map are served from our own site, so viewing a page does not send your information to font, map or video providers. The map is a static illustration built from OpenStreetMap data. If you click Get directions, Google Maps opens in a new tab and Google’s privacy policy applies there. Tapping a phone or email link opens your own phone or email app.

How we use information

  • To answer your questions and return your calls.
  • To coordinate consultations and physician referrals, and to verify insurance benefits when you ask us to. Once this involves your health information, HIPAA and our Notice of Privacy Practices apply.
  • To operate, maintain and secure the website, including detecting and preventing fraud, spam and attacks.
  • To comply with law and respond to lawful requests.

We do not use website information for automated decision-making or profiling, and we do not send marketing messages to people who contact us unless they ask to receive them.

How we share information

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We have not done so in the past 12 months.

We disclose information only as needed:

  • Service providers that host the website, deliver email and run our phone system, under contracts that limit their use of the information. Providers that handle health information on our behalf sign business associate agreements as HIPAA requires.
  • Your care team, such as your referring physician or insurer, when you ask us to or as permitted by HIPAA and the CMIA.
  • Legal and safety reasons: to comply with law, valid legal process or regulators, or to protect the rights and safety of patients, staff or the public.
  • Business changes: as part of a merger or transfer of the practice, subject to the same protections.

Your California privacy rights (CCPA/CPRA)

If you live in California, the California Consumer Privacy Act, as amended by the California Privacy Rights Act, may give you the following rights over personal information we hold about you. Medical information covered by HIPAA or the CMIA is handled under those laws instead, as described in our Notice of Privacy Practices.

  • Right to know and access the categories and specific pieces of personal information we have collected, where it came from, why we use it and who we disclose it to.
  • Right to delete personal information, subject to exceptions such as medical-record retention laws.
  • Right to correct inaccurate personal information.
  • Right to opt out of sale or sharing. We do not sell or share personal information, so there is nothing to opt out of. We also treat a Global Privacy Control signal from your browser as a valid opt-out request.
  • Right to limit use of sensitive personal information. We use sensitive information, such as health details you choose to send, only to respond to you and provide care, which are permitted purposes.
  • Right to non-discrimination. We will not deny service, charge different prices or provide a different quality of care because you exercised these rights.

Categories of information (last 12 months)

  • Identifiers (name, email, phone number, IP address): collected from you or your device to respond to you and secure the site; disclosed only to service providers.
  • Internet activity (server and security log data): collected automatically to deliver and protect the site; disclosed only to our hosting provider.
  • Sensitive information (health details), only if you include them in a message: used only to respond and to provide care.

How to make a request

Call (209) 670-1202 or email info@healthprtcl.com with the subject “Privacy request.” We will confirm receipt within 10 business days and respond within 45 calendar days (we may extend this once by up to 45 more days and will tell you why). We will verify your identity by matching information you provide with what we already have, and we will never ask you to create an account. You may use an authorized agent; we may ask for proof of the agent’s authority.

Shine the Light. We do not disclose personal information to third parties for their own direct-marketing purposes.

Children’s privacy

This website is meant for adults and is not directed to children under 13. We do not knowingly collect personal information from children through the site. If you believe a child has contacted us, please let us know and we will delete the information. Care for minors is arranged with a parent or legal guardian and is governed by health-privacy and consent laws.

Security and retention

The site is served only over encrypted HTTPS. We use reasonable administrative, technical and physical safeguards for the information we hold, and we limit access to staff who need it. No website or email system is completely secure, which is why we ask you not to send sensitive details by regular email.

We keep general correspondence only as long as needed to respond and for reasonable business records. Our hosting provider keeps log data for a limited period. Medical records are kept for the periods required by California law, and a request to delete website information does not by itself permit deletion of a medical record.

Changes to this policy

We may update this policy as the website or the law changes. The effective date at the top shows the current version. If we make a material change, such as adding analytics, we will post the update here before it takes effect.
